First off, who's eligible? If you're between 14 days old and 69 years, you're good to go—as long as you're traveling outside your home country. U.S. citizens and non-U.S. citizens can both sign up, but note that folks aged 65-69 get a maximum coverage of $100,000, while those under 65 can go up to $250,000 per injury or illness. No U.S. coverage for residents except for short incidental trips home.
The coverage kicks in the moment your application and payment are processed (online, or when you depart), and it lasts up to 364 days, with options to extend under certain rules. Premiums are flexible—if you pay monthly, you get refunds for unused whole months; if upfront, for unused days. But remember, no refunds if you've filed a claim!
Now, the heart of it: the benefits. With a $0 deductible and 100% coinsurance on eligible expenses up to your maximum limit, it's straightforward. For medical stuff, you're covered for hospital room and board (average semi-private rate with nursing), intensive care up to the max, and local ambulance rides if an illness or injury lands you in the hospital. In the U.S., there's a $100 co-pay for emergency room visits for illnesses (waived if admitted), and $50 for urgent care. Outside the U.S., no co-pays there!
Need some rehab? Up to $500 for outpatient physical therapy or chiropractic care. Ouch, a toothache? Emergency dental up to $1,000. Eye issues? Up to $150 for an emergency exam with a $50 co-pay. Even acute onset of pre-existing conditions is covered up to your max limit, plus $25,000 for emergency medical evacuation related to that.
Speaking of emergencies, travel benefits are a lifesaver: up to $100,000 lifetime for medical evacuation (not tied to the overall max), $20,000 for repatriation of remains, $10,000 for local burial or cremation, and $10,000 for crisis response like ransom or personal belongings in tough situations. Family support includes $50,000 for emergency reunion (up to 15 days), $1,500 for a bedside visit, $5,000 to return minor children home, and even $1,000 to get your pet back safely. Political evacuation? Up to $10,000 lifetime. Trip interruption covers up to $5,000 if you have to cut your journey short.
Accidents happen, so there's $25,000 lifetime max for accidental death and dismemberment for ages 18-69 (half for one limb), and lower for under 18. Common carrier accidents bump it to $50,000 for adults, $10,000 for kids, with a family max of $250,000. Lost luggage? Up to $3,000 per period ($500 per item, $6,000 lifetime). Travel delay? $100 a day after 12 hours, max 2 days. Lost passport? $100. Denied at the U.S. border on a B-2 visa? $500 help. Natural disaster messing with your stay? $100 a day for 5 days replacement accommodations. Hospital stays come with $100 per day indemnity, personal liability up to $25,000 (including third-party injury/property), and terrorism medical expenses up to $50,000 lifetime. Oh, and use the U.S. PPO network for direct billing and better rates inside the States.

Important notes: This isn't ACA-compliant, so check with a pro if that affects you. Pre-existing conditions are generally excluded except for acute onsets. Data privacy is tight—they only share what's needed legally. Claims must be filed within 60 days, appeals within 90. And general exclusions apply, like war, self-harm, or extreme sports unless specified.
